This has been an issue for a few patches now. File a bug report please. I've made topics on this before. Last time I filed a bug report and submitted a video they told me it was me erroneously pressing the button twice.

I have this on video a few times. I really just throw a remote. Every now and again it won't trigger. I just uploaded a video that it happens a few times in. I'll scrub through it and give you time markers for when it does happen.

Detonate directly? Not sure if I understand. Like when they're in view? It's happened at lots of ranges so far if thats what you mean. I'll try to detonate from the other side of the map then go back and vuala. They're still there.

As for the scanner plus shooting you can sprint cancel the scan, but your third person character will still have the scan animation going.

I believe in an IRC a dev said the explosion gets the damage multiplier of whatever the sphere touches first. If it blows your head off, its going to blow your head off.

I use both DS3 and KB/M. You change directions going ADADADAD much quicker/easier with kb/m than flicking a thumb stick. Don't care how fast your thumbs are the throw of keys is much much shorter.

Happens every day at least once a match. More common with semi auto weapons. It happens almost every time for MD's if there's a lot of enemies around.
